Job summary

Baystate Health in Springfield, MA is seeking a Part-time Case Management Assistant to support care coordination. You will work with patients and Case Managers to document clinical and regulatory information and collaborate with internal and external partners on financial advocacy.

The role includes chart audits for Medicare and coordination with insurers to ensure smooth transitions for post-acute care and scheduling follow-ups.

Qualifications

• High School Diploma or GED required; associate degree preferred.
• 2–5 years experience in a health-related field.
• Insurance/payer experience preferred.
• Basic knowledge of medical terminology.

Responsibilities

• Collaborate with patients and Case Managers to document clinical and regulatory information.
• Coordinate with internal and external partners on financial advocacy for the patient/organization.
• Audit charts for Medicare requirements and communicate regulations.
• Assist with follow-up appointments and post-acute/transitional planning care needs.
• Coordinate with insurance companies, accountable care organizations, home care agencies, SNFs, DME vendors, and transportation agencies.
