Job summary

Baystate Health is seeking a Part-Time Case Manager Assistant to join the inpatient Case Management Team at Baystate Medical Center in Springfield. You will collaborate with patients and Case Managers to document clinical and regulatory information, and support post-acute planning and coordination with insurers and care providers.

The role requires a High School Diploma or GED (Associate degree preferred) and 2–5 years in a health-related field, with insurance experience preferred.

Qualifications

• High School Diploma or GED required; Associate degree preferred.
• Two to five years of experience in a health-related field.
• Insurance/payer experience preferred.
• Basic knowledge of Medical Terminology.

Responsibilities

• Collaborate with patients and Case Managers to communicate and document clinical/regulatory information.
• Coordinate with internal and external partners for financial advocacy.
• Audit charts for Medicare requirements and communicate regulations.
• Assist with follow-up appointments and post-acute planning care needs.
• Coordinate with insurance, ACOs, home care, SNFs, DME, and transportation providers.
