Job summary

Liberty Hospital is seeking a Case Management Assistant to support the Case Management team under the direction of the Social Work Manager. The CMA will facilitate coordination of services for patients and families, and ensure confidential, timely communication with care teams during entry and inpatient processes.

The position emphasizes collaboration, accurate data handling, and efficient care coordination across the healthcare team, contributing to discharge planning and post-acute care

Qualifications

• High School graduate with 3+ years broad administrative/secretarial experience.
• Strong verbal and written communication skills.
• Excellent customer service and ability to manage multiple tasks.
• Knowledge of MS Office programs and data integrity.
• Confidentiality and ability to work with healthcare team.

Responsibilities

• Maintain confidentiality and privacy of patients, staff, and partners.
• Communicate promptly with case managers and staff about referrals.
• Route calls, messages, and healthcare correspondence to appropriate staff.
• Verify benefits with payers as requested by the case manager.
• Assist in coordination of referrals and discharge planning.
