Job summary

Baystate Health in Springfield, MA is seeking a part-time Case Manager Assistant to support the inpatient Case Management Team. You will collaborate with patients and Case Managers to document clinical and regulatory information and coordinate post-acute planning.

This role requires a High School Diploma or GED (Associate degree preferred) and 2–5 years in a health-related field. Insurance/payer experience and knowledge of medical terminology are a plus.

Qualifications

• High School Diploma or GED required; Associate degree preferred.
• Two to five years prior experience in a health-related field.
• Insurance/payer experience preferred.
• Basic knowledge of Medical Terminology.

Responsibilities

• Collaborate with patients and Case Managers to communicate and document accurate clinical and regulatory information.
• Collaborate with internal and external partners to address financial advocacy for the patient/organization.
• Audit Charts for Medicare requirements and communicate regulations effectively.
• Assist with follow-up appointments and telephone calls and other post-acute/transitional planning care needs.
• Support coordination with representatives from insurance companies, accountable care organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies.
