Job summary

Vibra Healthcare in Fresno, CA is hiring a Case Management Assistant to support the Case Management team with care coordination, discharge planning, and utilization review. You will gather information from the EMR and third-party payors, help secure authorizations, and document in the HMS system.

Responsibilities include arranging post-acute care, coordinating with providers, and following up on referrals and appointments after discharge.

Qualifications

• High school diploma or equivalent is required.
• Minimum one year of experience as an Administrative Assistant or higher.
• Experience in health care or health insurance setting is required.
• Significant computer/data gathering and processing skills are required.

Responsibilities

• Provide clerical and administrative support to the Case Management team to facilitate care coordination and discharge planning.
• Track concurrent review due dates and obtain updated clinical information from EMR/chart or forms for payors.
• Obtain authorizations and document in HMS financial system.
• Assist with arranging post-acute care and discharge referrals, including Home Health and Skilled Nursing facilities.
• Assist with post discharge physician appointments and follow-up calls as needed.
