Job summary

Universal Healthcare MSO, LLC is seeking a Case Management Assistant to support the Transitional Care Services team in Fresno, CA. This onsite role coordinates post-discharge services, gathers clinical information, and helps ensure smooth transitions for members within ECM.

You will work with Nurse Case Managers and Social Services to manage referrals, schedule appointments, and document care activities, while maintaining strong communication with members and providers.

Qualifications

• Education: High School diploma or GED required.
• Minimum of 3 years of experience in health care or community health settings.
• Advanced knowledge of prior authorization or case management regulations.
• Experience in a managed health care environment preferred.
• Medical Assistant or Community Health Worker certification preferred.

Responsibilities

• Assist Case Managers in coordinating transitional care services.
• Gather clinical information and coordinate post-discharge services.
• Enter data into CM systems and maintain accurate records.
• Communicate with members, providers, and agencies to support care transitions.
