Job summary

UofL Health is seeking a Case Management Assistant to coordinate care across the discipline, working under the direction of the Care Coordination Manager. You will collaborate with RN Case Managers, Social Workers, patients, and families to prioritize needs and streamline referrals.

The role emphasizes communication, confidentiality, and efficient discharge planning, ensuring timely post-acute transitions and adherence to policy. A healthcare background and CPR certification are required.

Qualifications

• High school diploma or equivalent is required.
• Minimum of one (1) year experience in a healthcare setting; hospital setting preferred.
• BLS-CPR certification is required.
• Kentucky Nurse Aide Certification is preferred.

Responsibilities

• Collaborate with RN Case Manager, Social Worker, patients, families, and the healthcare team to prioritize placement requests.
• Deliver MOON letter to Medicare and ensure patient understanding; document when provided.
• Discharge planning communication for Medicare patients and coordinate post-discharge needs.
• Meet with RN Case Managers and Social Workers daily to assess day’s needs.
• Coordinate discharge with internal/external providers while protecting patient information.
• Advocate for patient/family needs in a respectful, confidential manner.
• Maintain open communication with Case Managers and Social Workers about referrals.
