Job summary

UofL Health in Louisville, KY is seeking a Care Management Assistant to support the Care Coordination Department. Under supervision of the Manager/Director, the CMA collaborates with RN Case Manager, Social Worker, patients and families to coordinate placement and discharge planning.

You will handle notices like MOON letters, communicate with the care team, and help ensure timely referrals and post-acute arrangements.

Qualifications

• Education: High school diploma or equivalent required.
• Minimum of one year experience in a healthcare setting.
• BLS-CPR certification required; Kentucky Nurse Aide certification preferred.

Responsibilities

• Collaborate with RN Case Manager and Social Worker to prioritize placement requests.
• Coordinate discharge needs with internal and external providers.
• Maintain communication with case managers on referrals.
• Complete routine referrals and document in EMR.
• Protect patient information and maintain confidentiality.
