Job summary

Responsibilities

• Coordinates discharge needs with internal and external providers.
• Documents in EMR and participates in Quality Review Audit processes.
• Advocates for patient/family needs in a confidential manner.
• Acts as liaison for the interdisciplinary healthcare team to optimize care delivery.
• Educates physicians, patients, and staff on payors, documentation, and compliance issues.
