Job summary

Qualifications

• AS in Nursing must be actively pursuing Bachelor's Degree in Nursing, Business, Health Care Administration or equivalent.
• Bachelor's Degree completion is required within 3 years of date of hire; NLC accepted in lieu of Kentucky Nursing License.

Responsibilities

• Promotes optimal management of clinical resources by conducting timely initial admission assessments.
• Coordinates discharge needs with internal and external providers while protecting patient information.
• Performs documentation in EMR and supports quality review processes.
• Educates healthcare team about the role of RN: Case Manager and serves as resource to physicians and staff.
