Job summary

BayCare's Winter Haven Hospital is seeking a Case Management Assistant to help coordinate patient and family access to community services and discharge planning.

You will educate patients about the discharge appeal process, support transitions from inpatient care, and maintain frequent contact with patients, staff, and payor representatives.

Position requires strong customer service and communication skills; standard hours are Monday through Friday with some on-call or weekend support as needed.

Qualifications

• High school diploma or equivalent required.
• 1 year in medical-related field or 1 year customer service experience.
• Strong customer service and communication skills.

Responsibilities

• Assist with coordinating patient and family access to agencies, transportation, and services.
• Distribute regulatory documents to identified patients.
• Educate patients about the discharge appeal process.
• Support seamless transitions from inpatient care using hospital and community resources.
• Maintain frequent contact with patients, families, hospital staff, and payor representatives.
