Job summary

BayCare is seeking a part-time Care Coordination Specialist in Winter Haven to join an interdisciplinary team focused on value-based care delivery. You will coordinate comprehensive care planning, ensure smooth transitions, and optimize outcomes by integrating clinical, social, and economic determinants of health.

The role emphasizes collaboration with patients, nursing, physicians, and community partners, with a 16-hour weekly schedule (Saturday and Sunday 8:30 AM - 5:00 PM).

Qualifications

Bachelor's degree in social work, psychology, or healthcare related field or HS graduate with LPN license.
Medical Social Work or LPN experience preferred

Responsibilities

Coordinate care across interdisciplinary teams to optimize outcomes.
Lead discharge planning and ensure post-acute resource utilization.
Address barriers including social determinants of health to improve transitions of care.
Coordinate services, referrals, and transportation to ensure continuum of care.
Support compliance with regulatory requirements and value-based care goals.
