Job summary

BayCare Health System in Largo, FL is seeking a Care Coordination Assistant (PRN) to support referrals and patient information in EMR systems. You will verify benefits, secure authorizations, and ensure accurate payer coding and financial data entry.

This role works under the Care Transition Coordinator or RN Case Manager to help maintain positive relationships with patients and payors. The position offers weekday hours, occasional weekends, and a supportive team in a community-focused health

Qualifications

• 1 year of Customer Service experience
• 1 year of Insurance Verification experience
• Experience in a Medical-related field preferred

Responsibilities

• Enter new referrals and update patient information in EMR systems including demographics, insurance, and benefit details.
• Verify insurance and benefits for Medicare, Managed Care and Private Plans; secure required authorizations.
• Select correct payer codes to ensure accurate intake information.
• Enter financial information (benefits, deductibles, co-pays, coinsurance, out-of-pocket) into the EMR.
• Timely request and communicate authorizations to payers and document in EMR.
• Work under the direction of the Care Transition Coordinator or RN Case Manager to maintain positive relationships with patients, payors, and other customers.

The Care Coordination Assistant Responsibilities Include

• Entering new referrals and updates all existing patient information in corporate IS systems including demographics, insurance, and benefit information.
• Verifying insurance and benefits for all Medicare, Managed Care and Private Fee for Service Plans. Securing authorization for services provided by BayCare HomeCare from the payers.
• Selecting the correct payer codes to provide accurate intake information.
• Understanding medical criteria for securing all required information from the referral source.
• Entering the appropriate financial information including benefits, deductibles, co-pays, coinsurance, out-of-pocket, another related costs into the EMR system.
• Timely requesting and communicating of authorizations to payer and documentation in EMR.
• The Care Coordination Assistant will work under the direction of the Care Transition Coordinator or RN Case Manager to establish and maintain a positive working relationship with patients, payors and other customers.

• Schedule: 8 :00 a.m. - 5:00 p.m., Monday – Friday
• Weekend requirement: Occasional

Education

• Required High School Graduate or Equivalent GED
• Preferred Technical Diploma 0Medical Specialization

Experience

• Required 1 year Customer Service
• Required 1 year Insurance Verification
• Required 1 year Medical Related field
