Job summary

Lehigh Valley Health Network, Inc. is seeking a Care Continuum Partner to join the Muhlenberg team in Bethlehem. This full-time Day Shift role partners with clinicians to coordinate appointments, referrals, and transitions of care for patients across the continuum.

You will support patient outreach, scheduling, and care plan execution, using the patient portal and other platforms to ensure a seamless experience in a busy physician practice.

Qualifications

• High School Diploma/GED required; emphasis on accuracy and confidentiality.
• 2 years experience interacting with patients in healthcare settings.
• 3 years experience in a complex health system or related field.

Responsibilities

• Collaborates with clinical care team to support pre-visit planning and patient flow.
• Coordinates complex appointments across continuum of care (procedures, visits, tests).
• Manages referrals, intake, scheduling, pre-certification, and follow-up communications.
• Serves as a patient concierge for regional or practice models and supports care plan compliance.
• Communicates with patients via telephone, portal, and other platforms; explains preparation for tests and visits.
