Qualifications

• Associate’s Degree from an Accredited School of Nursing as an RN, 3 years Recent experience in a clinical setting.
• Ability to initiate, accept, and adapt to change within a healthcare setting.
• Ability to take initiative and function independently, as well as collaboratively.
• Excellent computer skills including EMR, Word, PowerPoint, and Excel.
• Time management skills.

Responsibilities

• Oversees clinical operations and facilitates patient engagement through visit flow, clinical triage, EMR clinical intelligence tools, pre-visit planning, transitions of care follow-up, and other patient focused initiatives to enhance the patient care experience.
• Prepares daily charts and maintains medical files and electronic medical records.
• Assesses clinical staff competencies and provides remediation, as necessary, in collaboration with Clinical Services.
• Facilitates coordination of care across settings for a designated population and ensures that labs, studies, and referrals are completed and results are discussed with the patient.
• Provides regular feedback and involves other colleagues in quality improvement workflow development.
• Reviews clinical staff competencies and provides remediation, as necessary.
