Qualifications

• 1-2 years of recent acute care hospital experience required.
• Data entry/computer experience required.
• RN license or equivalent credential required to practice.

Responsabilités

• Coordinates patient care progression throughout the continuum of care.
• Ensures plan of care is patient-focused, high quality, efficient and cost-effective.
• Meets with patient/family to assess needs and develop an individualized continuing care plan.
• Monitors length of stay and resource use aiming for continuous improvement.
• Communicates with Utilization/Case Management and physician teams for appropriate care and reimbursement.
• Supports patient flow and implements processes to reduce delays.
• Advocates for ED patients, coordinating services for complex cases.
• Ensures critical plan-of-care elements are communicated to patient/family and team.
• Performs other duties as requested by Case Manager Supervisor/Director.
