Job summary

Medical University of South Carolina invites qualified RNs to join as Nurse Case Manager I. Reporting to the Manager of Case Management and Care Transitions, you will oversee the plan of care across the care continuum to ensure efficient patient flow through the hospital.

Under guidance of the Team Leader, you coordinate discharge planning, resource utilization, and case management as a clinical expert. This is a salaried 40-hour-week role with a focus on safe transitions.

Qualifications

• Bachelor of Nursing degree preferred with minimum of one year of experience.
• RN licensure by the state of South Carolina or compact state required.
• Current AHA BLS or Red Cross BLS for Healthcare Providers certification required.
• Case Management certification preferred.
• RN hired with ADN/ Diploma must enroll in BSN within two years and complete BSN within four years.
