MLK Community Hospital is seeking a Case Manager to support physicians and the interdisciplinary team in facilitating patient care and optimizing outcomes while managing costs. The role includes utilization management, care progression and discharge planning to ensure timely and appropriate care across the continuum.

The Case Manager manages a designated patient caseload, conducts comprehensive assessments, coordinates precertification and payer interactions, and collaborates with medical staff

Formación

• Bachelor of Science degree in nursing preferred.
• Minimum of 1–3 years hospital or related experience.
• Able to navigate outside provider networks (Health Plans, IPA's, and FQHC's).
• Current California Nursing license.
• BLS certification required.
• Certification in Case Management preferred.

Responsabilidades

• Completes comprehensive assessment to identify intervention opportunities for psycho-social, cultural, spiritual and physical care needs.
• Coordinates precertification and payer authorization processes.
• Facilitates transfers to appropriate care settings and discharge planning.
• Collaborates with physicians and the interdisciplinary team to ensure timely, cost-effective care.
