Job summary

Responsibilities

• Coordinates case management from entry to post-discharge across outpatient settings.
• Identify patients suitable for case management based on cost, complexity, or request.
• Monitor and document quality of care and effectiveness of care plans.
• Assess funding and insurance issues to optimize utilization of services.
• Plan timely discharges with the healthcare team.
• Oversee CMS Important Message issuance and supervise staff assigned to case management teams.
