Job summary

Responsibilities

• Coordinates case management from entry to post-discharge and coordinates with physicians, social workers and team members.
• Monitors and documents quality of care, assesses patient needs and ensures care plans are followed.
• Negotiates with third party payers and discusses funding and insurance issues to optimize utilization.
• Establishes discharge plans and follow-up appointments.
• Oversees CMS Important Message issuance and supervises case management staff.
