Job summary

ChenMed is seeking a Patient Continuity and Needs Advocate (PCNA) to bridge patients with tailored healthcare plans and support services. In this role you will help maintain strong patient relationships, ensure seamless care transitions, and assist with documentation using EMR systems, Medals, and Salesforce.

Ideal candidates will have strong English communication, a background in healthcare benefits, and a minimum of three years’ experience, preferably with seniors and Medicare Advantage

Qualifications

• Proven customer service skills and ability to build trust with patients and partners.
• Understanding of Medicare Advantage and healthcare benefit services.
• Experience with EMR systems and handling patient documentation.

Responsibilities

• Evaluate patient needs and connect with health plans and resources.
• Proactively engage disenrolled members to support back-to-care efforts.
• Collaborate with center leadership, brokers, and clinical staff on retention strategies.
