Job summary

Central Florida Family Health Center Inc. in Sanford, FL is seeking a Primary Care Access Network Navigator - Hospital Based to support uninsured patients through the PCAN program.

The navigator will educate, screen eligibility, and link clients to Medicaid/Medicare and community services; interview and document applications; and ensure timely follow-up on enrollment outcomes. On-site position with strong people skills required.

Qualifications

• Associates Degree from an accredited institution.
• Two years of experience in social, health, or public services, or an equivalent combination of relevant education, training, and experience.
• Proficient with word processing and spreadsheet applications; strong computer literacy.

Responsibilities

• Provide outreach/education to patients, families, caregivers, social workers/case managers in designated zip code areas.
• Interview potential clients and determine eligibility for participation according to program guidelines.
• Exercise independent judgement in eligibility determination and link to Medicaid/Medicare programs.
• Ensure regular follow-up with clients to determine enrollment outcomes.
• Answer routine questions and respond to inquiries in a timely manner.
• Attend PCAN Case Management and school nurse meetings as scheduled.
• Compile files, maintain logs, and assist with scheduling appointments.
• Prepare monthly reports outlining program statistics.
• Other responsibilities as assigned.
