Job summary

Qualifications

• Associates Degree from an accredited institution of higher learning.
• Two years of experience in social, health, or public services or equivalent.

Responsibilities

• Provide outreach/education to patients and families in designated areas.
• Interview potential clients to determine eligibility for program participation.
• Exercise independent judgement in eligibility and financial status assessment.
• Complete paperwork for eligible clients into Medicaid/Medicare programs.
• Ensure timely follow-up on enrollment outcomes for clients.
• Attend and participate in PCAN-related workgroups and meetings.
• Compile files, maintain logs, and assist with scheduling.
• Prepare monthly reports on program statistics.
