Job summary

Northwestern Medicine Central DuPage Hospital is seeking a BCVI Clinical Nurse Coordinator to lead care coordination for cardiovascular patients. The role includes direct patient care, coordinating referrals, and organizing clinical conferences with the cardiovascular team.

The BCVI Clinical Nurse Coordinator will manage data to drive clinical workflows, coordinate care pathways, and ensure compliance with hospital policies. Travel between Delnor and Kishwaukee is intermittently required.

Qualifications

• Bachelor’s degree in nursing required.
• Current IL nursing license required.
• Current Basic Life Support certification required.

Responsibilities

• Plan, coordinate, direct, and provide appropriate clinical care to patients.
• Collaborate with interdisciplinary team to coordinate care and optimize wellness.
• Educate patients, families, and staff; serve as a resource for ongoing education.
