Job summary

Northwestern Medicine’s BCVI is seeking a Clinical Nurse Coordinator to lead the care plan for designated cardiovascular patients, coordinating referrals, screenings, tests, and consults with the cardiovascular team.

The role requires a Bachelor’s degree in nursing, an active IL nursing license, BLS certification, and 2–5 years in cardiovascular surgery or cardiology; travel to Delnor and Kishwaukee will be required, with main duties at CDH.

Qualifications

• Bachelor’s degree in nursing.
• Current IL nursing license.
• Current Basic Life Support certification.
• 2–5 years experience in cardiovascular surgery and/or cardiology.
• Experience working in hospital and/or clinic setting with direct physician interaction.

Responsibilities

• Plan, coordinate, direct, and provide appropriate clinical care to designated patients.
• Collaborate with interdisciplinary team to optimize care and cost effectiveness.
• Develop and provide education for patients, families, and staff.
• Serve as a communication link among patients, families, and team.
• Coordinate follow-up visits, referrals, testing, and long-term care.
• Schedule and prepare clinical conferences and presentations as needed.
