Job summary

Northwestern Medicine Central DuPage Hospital is seeking a BCVI Clinical Nurse Coordinator to support our hypertension program across the network. This role, based primarily at CDH, may require travel to other NM locations and AMC to coordinate care for cardiovascular patients.

The ideal candidate holds a Bachelor of Science in Nursing, an IL nursing license, and BLS certification, with 2–5 years of cardiovascular surgery or cardiology experience.

Qualifications

• Excellent interpersonal and communication skills required for patient education.
• Possesses computer skills to prepare spreadsheets and enter data.

Responsibilities

• Plan, coordinate, direct, and provide appropriate clinical care to patients in the designated care setting.
• Collaborate with the interdisciplinary team to coordinate ongoing care and maximize wellness.
• Develop, coordinate and provide education for patients, families, caregivers and staff.
• Serve as a communication link between patient, family and team.
• Coordinate comprehensive care including follow-up visits, referrals and tests.
• Schedule and coordinate clinical conferences; prepare content and presentations.
