Job summary

Northwestern Medicine seeks a BCVI Clinical Nurse Coordinator to support our system hypertension program, primarily at CDH with travel to other health network locations and AMC. The role blends direct patient care with care coordination, referrals, and education.

Qualifications

• Education: Bachelor's degree in nursing.
• License/Certification: Current IL nursing license.
• License/Certification: Current Basic Life Support certification.
• Experience: 2-5 years in Cardiovascular surgery and/or Cardiology.
• Experience: Hospital/clinic setting with direct Physician interaction.

Responsibilities

• Plan, coordinate, direct, and provide clinical care to designated patients.
• Coordinate ongoing care with the interdisciplinary team to maximize wellness.
• Develop and provide education for patients and families and staff.
• Serve as a communication link among patient, family, and team.
• Coordinate follow-up visits, referrals, testing, and long-term care.
