Responsibilities

• Receives incoming referral requests from physicians/providers.
• Verifies eligibility and reviews patient demographics.
• Engages patients by telephone to discuss referrals and appointment needs.
• Coordinates referral requests with physicians/providers, specialty practices and other consultants.
• Schedules patient appointments in the EMR or by contacting specialty practices.
• Confirms appointments and referrals with physicians/providers and patients.
• Maintains timely documentation and data collection in the EMR.
• Supports department teammates to meet turnaround time expectations.
