Job summary

Advocate Health is seeking an experienced RN Navigator in Cardiology/Electrophysiology to coordinate care from referral through follow-up. This role supports patients undergoing ablation and related procedures, acting as a consistent clinical resource across teams to promote timely access and a seamless patient experience.

BSN is required with MSN preferred; current RN licensure for the state is essential, along with certification in the appropriate field within two years.

Qualifications

• BSN required; MSN preferred.
• Current RN license in the state of practice.
• Certification in the appropriate specialized field within 2 years.
• Excellent communication skills required.

Responsibilities

• Coordinate patient care from diagnosis through follow-up to improve efficiency and quality.
• Educate patients and families; address barriers to care.
• Coordinate pre-procedure testing, anticoagulation, and medication plans.
• Provide post-procedure follow-up and symptom triage; escalate clinical concerns.
• Support Electrophysiology procedures, particularly ablation pathways, across care settings.
