Job summary

The University of Kansas Health System is seeking an Ambulatory Nurse Navigator (RN) to lead patient care coordination in Cardiology. The navigator will assess needs before visits, document interventions in the EMR, coordinate referrals, provide education, and ensure timely scheduling with the appropriate multidisciplinary team.

Ideal candidates have a BSN with 3+ years of nursing experience and experience in ambulatory care or case management.

Qualifications

• RN with active license and BLS required.
• Minimum BSN; MSN preferred for advancement.
• Experience in ambulatory care coordination or case management is preferred.

Responsibilities

• Assess new patient needs before first visit to identify support services.
• Document thoroughly in EMR and track interventions.
• Coordinate referrals to specialty programs and education resources.
• Provide education and emotional support throughout intake and treatment planning.
• Ensure scheduling with appropriate providers and locations.
