Job summary

CarolinaEast Health System is seeking a Patient Navigator to coordinate patient care and serve as a clinical resource throughout the care journey. You will support referrals, communication, and triage to promote continuity of care across the practice.

Ideal candidates have RN licensure with at least 2 years in cardiology or electrophysiology and strong relationship-building and communication skills to coordinate care among departments.

Qualifications

• RN licensure required (state nursing license).
• Associate's degree in nursing required; Bachelor's degree preferred.
• Minimum 2 years nursing experience, preferably in cardiology or electrophysiology.
