Job summary

Atrium Health is seeking a Nurse Navigator to coordinate arrhythmia patient care from diagnosis through follow-up. You will act as a patient advocate, coordinating across electrophysiology, imaging, anesthesia, pharmacy, scheduling, and referring clinicians to promote timely access and a seamless patient experience.

Qualifications

• BSN degree required; MSN preferred.
• Current licensure to practice as a Registered Nurse in the state where the work is performed.
• Certification in the appropriate specialized field within 2 years of being in the position required.
• Excellent communication skills required.

Responsibilities

• Coordinates care across Electrophysiology teams, imaging, anesthesia, pharmacy, scheduling and referring clinicians.
• Educates patients and families about diagnosis, work-up, treatments and follow-up.
• Serves as patient advocate and liaison to ensure resources and services are provided.
• Assesses barriers to care (insurance, transportation) and coordinates referrals.
• Contacts newly diagnosed patients at the time of diagnosis.
• Promotes continuity of care across inpatient and outpatient settings.
