Job summary

Carle Health in Illinois is seeking an LPN to work with RNs on high-acuity patients. The role includes administering medications, assisting with IV piggybacks, trach care, dressing changes, and documenting assessments and care plans after the RN has begun initial evaluation.

You will pair with UNL as needed and coordinate with physicians/APPs to receive orders, support discharge education, and ensure accurate charting.

Qualifications

• Licensed Practical Nurse with current Illinois license (IDFPR).
• BLS certification within 30 days (AHA).
• College diploma: Licensed Practical Nurse required.

Responsibilities

• Passes medications, including oral meds and IV piggybacks (excluding IV Push).
• Charts assessments on assigned patients after RN has completed initial assessment.
• Charts on care plans opened by the RN; assist with new care plans as needed.
• Communicates with physicians/APPs to receive orders and coordinate care.
• Complete dressing changes, trach care, NG tube care and feedings.
• Care for ostomy and reinforce discharge instructions.
