Job summary

You will chart assessments and participate in care planning after the RN completes the initial assessment and initiates the care plan, coordinating with physicians for orders and patient care.

Qualifications

• Licensed Practical Nurse with active IL license (IDFPR).
• Current Basic Life Support (BLS) certification within 30 days of hire.
• College diploma in nursing with LPN credential.

Responsibilities

• Passes medications and hangs IV piggybacks and fluids (excluding IV push).
• Charts assessments after RN initial assessment and assists with care plans.
• Coordinate with physicians/APPs for orders and care.
• Perform dressing changes, trach care, NG tube care and feedings.
• Care for ostomy and reinforce discharge instructions.
