Job summary

Carle Health is seeking an LPN to support high-acuity patients, coordinating with RNs and UNLs to manage patient care. Responsibilities include medication administration, IV piggybacks, trach care, and helping with care plans after the RN's initial assessment.

Responsibilities expand to dressing changes, NG tube care, ostomy care, and reinforcing discharge instructions while communicating with physicians and APPs for orders.

Qualifications

• Current LPN license (Illinois or applicable state).
• BLS within 30 days of hire.
• College diploma: Licensed Practical Nurse (Required).

Responsibilities

• Administer oral meds and IV piggyback fluids per protocol.
• Chart assessments on assigned patients after RN initial assessment.
• Assist RN with care planning and follow-up per orders.
• Perform dressing changes and trach care as needed.
• Insert and care for NG tube and feedings; ostomy care.
• Reinforce discharge education and instructions; communicate with physicians/APPs for orders.
