Job summary

Qualifications

• Team members will maintain annual educational requirements for hospital based certifications (e.g. DNV, CARF or others).
• GED/HSED or High School Diploma, or High School Graduate.

Responsibilities

• Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation.
• Measures and documents vital signs and patient data in EMR.
• Performs purposeful rounding and communicates observations to RN.
• Participates in admission, transfer and discharge processes under supervision of RN.
• Collects urine, stool and sputum specimens and performs point-of-care testing.
• Delivers trays, charts, specimens and reports; maintains stock for nursing area.
• Performs patient positioning and transporting duties; assists with diet review and tray setup.
• Maintains skin integrity and uses TED hose and sequential compression devices.
• May remove IVs, provide post mortem care, and bladder scans as directed.
• Follows infection prevention policies; proper hand hygiene and attire.
• Assesses age-specific needs and provides appropriate care.
