Qualifications

- 2+ years in social work and 2+ in care management preferred.

Responsabilités
- Provide grief counseling, crisis intervention and discharge planning support.
- Assess patients and families for discharge planning needs across inpatient, observation and ED.
- Develop discharge plans with contingencies to ensure timely care coordination.
- Communicate with payors for post-acute care authorizations as needed.
