AdventHealth Palm Coast seeks a skilled Social Worker to support patients and families with discharge planning and care coordination. You will collaborate with a multidisciplinary team to assess needs and mobilize community resources while advocating for patient rights.

Qualified candidates hold an MSW and have 4+ years of social work experience, with familiarity in care management and crisis intervention. This PRN role supports compassionate, holistic care in a faith-based health system.

Qualificações

• MSW required and licensed according to state rules.
• 4+ years experience in social work.
• Care management experience preferred.
• ACM/CCM licensure preferred.

Responsabilidades

• Educate patients and families on care options and discharge planning.
• Assess readmitted patients and coordinate multidisciplinary care.
• Develop discharge plans and coordinate post-acute services.
• Provide grief counseling and crisis intervention as needed.
