AdventHealth in Wesley Chapel, FL seeks a Care Management Specialist to support discharge planning and post-acute care coordination. You will assess inpatient and emergency department needs, develop discharge plans, and coordinate with post-acute services to ensure timely and safe transitions for patients.

Qualified candidates have a BSW and preferably a Master’s in social work, with 2+ years in care management or social work.

Qualifikationen

• 2+ social work experience required.
• 2+ care management experience preferred.
• BSW is required; Master’s preferred.

Aufgaben

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate.
• Assesses patients’ and families’ wholistically for discharge planning needs in the inpatient, observation and/or emergency departments.
• Reviews the medical record and integrates clinical, social, and financial factors into the transition of care plan.
• Develops discharge plans with contingency plans to ensure timely care coordination and post-acute care arrangements.
• Coordinates with post-acute care services and facilities to ensure continuity of care and accurate discharge reconciliation.
