Job summary

Qualifications

• Bachelor's degree in Nursing required.
• 3+ years of nursing experience.
• 1+ years of case management or ambulatory care coordination experience.

Responsibilities

• Assess needs for new patients referred to a specialist prior to their first visit to identify the need for support services or other resources.
• Document patient assessments and interventions in the EMR.
• Refer new patients to the program’s supportive services based on navigation needs assessment.
• Provide educational and emotional support throughout intake and treatment planning, remaining a resource throughout care.
• Coordinate scheduling with multidisciplinary providers at appropriate locations/timelines.
• Collaborate with intake scheduler to ensure complete medical information for the first appointment.
