The University of Kansas Health System is seeking an Ambulatory RN Care Coordinator for the Hematology clinic at Westwood Medical Pavilion in West. This full-time role supports physicians by coordinating patient care, triaging visits, educating patients, and ensuring timely communication with families and the care team.

The RN will use EMR tools to document assessments, coordinate services, monitor outcomes, and maintain patient confidentiality while delivering compassionate and evidence-based

Qualifications

• Bachelor degree in Nursing required
• 2+ years RN experience
• BLS certification within 14 days of hire

Responsabilités

• Manage patient flow, triage, and direct patient care.
• Liaise between front desk, nursing staff, physicians, and clinic manager.
• Coordinate care with providers to set patient goals and optimize outcomes.
• Provide nursing support/coordination to the practice to improve patient quality of life.
• Educate patients and families about health care needs and treatment plans.
• Document assessments and plans in EMR; ensure orders are completed and communicated.
