Job summary

Qualifications

Responsibilities

• Assess needs for new patients referred to a specialist prior to their first visit.
• Document patient assessment and interventions in the EMR.
• Refer patients to the specialty program's supportive services.
• Provide educational and emotional support throughout intake and treatment planning.
• Coordinate with the intake scheduler to ensure medical information is ready for first appointment.
• Develop relationships with referring physicians and ensure follow up and treatment plan communication.
• Complete intake logs and report monthly for outcome metrics.
• Participate in quality improvement initiatives and hospital committees.
