Job summary

Advocate Christ Medical Center is seeking a dedicated addiction services case manager to coordinate biopsychosocial assessments, develop diagnostic summaries, and formulate individualized treatment plans. You will lead group therapy and collaborate with patients, families and the care team to support recovery goals.

The position carries a 36-hour work week with 8:00 am to 8:30 pm shifts and requires completion of clinical supervision for students and interns where applicable.

Qualifications

• Associates Degree in Behavioral Health or equivalent; Bachelors preferred.
• 2 years of experience in the field.
• Must recognize needs and behaviors of diverse age groups.

Responsibilities

• Responsible for case management of addiction patients and families.
• Completes biopsychosocial assessments of patients.
• Formulates diagnostic summaries and individualized treatment plans.
• Coordinates team input and goals in treatment planning.
• Delivers daily group therapy using group dynamics.
