Job summary

Advocate Health is seeking an experienced addiction counselor to manage casework for patients and families. You will complete biopsychosocial assessments, formulate diagnostic summaries, and develop individualized treatment plans in collaboration with the care team and family when appropriate.

You will conduct addiction-focused group counseling, supervise students under the Substance Use Service, deliver education on addiction and recovery, and contribute to quality improvement initiatives.

Qualifications

• Associates degree in Behavioral Health or equivalent; Bachelor's degree preferred.
• Minimum 2 years of experience in the field.
• Ability to recognize the needs and behaviors of a variety of age groups.

Responsibilities

• Case management for addiction patients and families, including biopsychosocial assessment and diagnostic summary.
• Formulates individualized treatment plans based on identified clinical needs.
• Coordinates team input and family/patient goals when formulating plans; revises plans as progress dictates.
• Introduces patient and family to 12-step recovery; prepares discharge plans with the team.
• Delivers addiction-focused group counseling using peer-centered approaches and group dynamics.
• Clinically supervises students and supports staff development in addiction treatment principles.
• Delivers education on addiction and recovery; contributes to quality improvement initiatives.
