Job summary

National Youth Advocate Program is seeking a full-time Bridges Liaison in Ohio to support young adults aged 18–21 exiting foster care. You will oversee eligibility, enroll participants, coordinate housing and care management, and work with courts as needed.

The role emphasizes independent living skills, timely referrals, and ongoing case documentation in CCWIS, with a caseload of 12–15. A Bachelor’s in social work or related field and 2 years’ child welfare experience are required.

Qualifications

• Bachelor’s degree in social work or related field is required.
• Two years of experience working with families in child welfare.
• Strong advocacy and collaboration across teams and partners.
• Must be reliable with transportation and driving record.

Responsibilities

• Enroll Bridges participants and maintain eligibility in CCWIS for IV-E.
• Complete Bridges assessments and monthly plan updates.
• Coordinate housing and care management needs for young adults.
• Make timely referrals for housing/case management services.
• Meet with each young adult at least monthly; attend court as needed.
• Document case notes and participate in quality improvement activities.
