Job Overview

Primary Care Nurse Practitioner / Physician Assistant position located in the Boston, Massachusetts area. The role is permanent and supports a Medicare‑focused patient population (65+).

Responsibilities

• Provide primary care for 15–18 patients per day on a hybrid schedule: four onsite days (8:00AM–5:00PM) and one remote day each week.
• Maintain a light call schedule (≈ two evenings per month and approx. two weekends per year).
• Coordinate care with specialists and hospitals; use Athena EMR.
• Deliver comprehensive, team‑based primary care focused on chronic disease management, preventive care, and social support.
• Address social determinants of health including housing, food security, and social isolation.
• Collaborate with physicians and APPs in a team‑based setting.

• Board certified or truly board eligible Nurse Practitioner or Physician Assistant.
