Job summary

Merakey is seeking a Blended Case Manager for its program in Philadelphia, PA. The BCM develops, coordinates, and implements services for assigned consumers per their service plan, addressing behavioral and physical health, family functioning, education and vocational goals, and socialization.

The role requires a relevant bachelor's degree, driver’s license, and may prefer experience with children in human services; bilingual abilities are a plus.

Qualifications

• Experience in the human services field working with children preferred.

Position Overview

The Blended Case Manager (BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The BCM will work and assist their assigned consumers by focusing on the following life domains: behavioral and physical healthcare needs, family functioning/independent living, educational and vocational goals, and socialization.
