Job summary

Merakey is seeking a full-time Case Manager to join the Dual Diagnosis Treatment Team in Greensburg/Indiana, PA. The role focuses on assessing needs, developing recovery plans, and coordinating services for adults and youths with mental health diagnoses or autism spectrum disorders.

You will work collaboratively with a treatment team in diverse settings, provide intensive therapeutic support, and help individuals access community resources while promoting person-centered care.

Qualifications

• Bachelor’s degree in social work, psychology, or another human service-related field required.
• Valid Pennsylvania driver’s license with a clean driving record and reliable transportation required.
• Experience in the behavioral health field working with individuals with mental health or developmental disabilities.

Responsibilities

• The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or autism spectrum. They provide intensive therapeutic services in all types of settings, including community living arrangements, schools, community settings, natural support residents, etc. In collaboration with other treatment team members, the Case Manager implements and transfers skills identified in the recovery treatment and support plan using a person-centered approach.

Position Details
