Job summary

Merakey is seeking a full-time Case Manager to join our Dual Diagnosis Treatment Team in Dubois, PA. You will assess, plan, and support recovery for adults and youth with mental health needs, coordinating care across community and school environments.

The ideal candidate will have a bachelor’s degree in a human service field, one year of behavioral health experience, and a willingness to travel in the community. Merakey offers comprehensive benefits and growth opportunities.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or another human service-related field required.
• Will be required to complete clearances and background checks, at Merakey’s expense.
• Valid PA driver’s license with a clean driving record required.
• 1 year of experience in the behavioral health field required.
• Candidate must be open to community based travel.

Responsibilities

• The Case Manager assesses, plans, and supports recovery treatment and plans for adults and youth with mental health diagnoses or autism spectrum considerations.
• Provides intensive therapeutic services in settings such as community living arrangements, schools, and community settings.
• Collaborates with the treatment team to implement skills in the recovery plan using a person-centered approach.

Position Details

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. This candidate, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.
