Job summary

Qualifications

• Must be bilingual (Spanish/English).
• Bachelor's degree in social work, psychology, or a human services-related field OR equivalent experience.
• Three years of experience working with at-risk youth, preferably homeless populations.
• Familiarity with social services and District agencies.
• Strong communication and organizational skills; exempt, full-time position.

Responsibilities

• Provide care coordination and social service navigation for clients.
• Develop and monitor individual service plans with a competency-based framework.
• Connect clients with social services, document progress, and assist with vital documentation and benefits.
• Conduct individual counseling and support life skills/workshops.
• Maintain confidential case files and timely service documentation (24–48 hours).
• Participate in team meetings, trainings, and community outreach; some evenings/weekends required.
